Appointments & Fees Schedule

Office hours are by appointment only.  To make an appointment, please call my clinic at 203-738-0020. For your convenience, a  questionnaire is provided for you to print out (see menu at left). Please complete it, and mail, fax, or bring it to the clinic at least two days before your appointment.

On your initial visit, please bring copies of your most recent laboratory results from other health care practitioners, if available.  Please also bring a bag with everything you are currently taking, whether prescription medications or over-the-counter supplements.
